Overview

Join our healthcare team as a Nurse Practitioner, where you will play a vital role in delivering exceptional patient care.

We are looking for an NP to work roughly 2 days a week (but flexible) seeing a variety of patients in a primary care setting. Preference is Tuesday, Wednesday afternoon/evening, Fridays or weekends. Also possibility of some work from home doing phone calls could be considered.

Our family medicine office is a supportive environment and most shifts there would be 1-2 doctors present for help if needed.

Duties

- Conduct comprehensive patient assessments, including history taking and physical examinations for children, seniors, and adults.
- Develop and implement personalized treatment plans, including medication management and pain control strategies.
- Collaborate with multidisciplinary teams using electronic health record PS Suites.
- Provide case management services to support patients with complex health needs, including dementia care and assisted living support.
- Educate patients and families on health conditions, medication use,



lifestyle modifications, and preventive care practices.
- Document all clinical encounters thoroughly while maintaining compliance with healthcare regulations and standards.

Requirements

- Valid Nurse Practitioner license with current certification to practice independently.
- Proven experience working with children (pediatrics) and senior populations in various healthcare environments such as hospitals or nursing homes.
- Experience using electronic health record PS Suites preferred but not mandatory.
- Excellent patient care skills combined with compassionate communication tailored to diverse patient needs.
