Monarch Health Solutions Inc. is seeking a dedicated and experienced Occupational Therapist (OT) to join our growing in-home healthcare team in Victoria, BC. This role focuses on delivering high-quality, person-centered care to seniors in their homes. The OT will complete assessments, create treatment plans, and support clients in maintaining independence through our Ageing-in-Place program.

Responsibilities

- Provide in-home occupational therapy to clients in Victoria and surrounding areas
- Complete comprehensive assessments, including mobility, ADLs, cognition, and home safety
- Develop individualized treatment plans aligned with client goals
- Recommend adaptive equipment and environmental modifications
- Educate clients, families, and caregivers on therapy techniques and protected mobility
- Document assessments, interventions, and progress notes accurately and on time
- Collaborate with Monarch’s interdisciplinary team to support client wellness
- Monitor client progress and adjust treatment plans as needed




- Maintain professional and ethical standards at all times

Qualifications

- Minimum 2 years of experience as an Occupational Therapist
- Master’s degree (or equivalent) in Occupational Therapy
- Current registration and good standing with the College of Occupational Therapists of British Columbia (COTBC)
- Valid driver’s license and reliable vehicle (required for home visits)
- Strong assessment, communication, and documentation skills
- Experience working with seniors or in community/home-based settings is an asset
- Ability to work independently with excellent clinical judgment
