Palliative Care Clinical Coach / Nurse Navigator

Hospice Huronia · North Simcoe Ontario Health Team · Georgian Bay General Hospital

Contract (to March 31, 2027) · 37.5 hrs/week, Mon–Fri · North Simcoe, on Georgian Bay

Three organizations. One bold goal: transform palliative care across North Simcoe. We're looking for an experienced RN/RPN who loves coaching others and driving real system change not just delivering care, but by multiplying its impact through every provider they mentor.

You'll split your time between hands-on nurse navigation and coaching front-line teams across a close-knit network of hospital, hospice, and home care serving a vibrant community of 53,000, including Francophone, First Nation, and Métis communities, against the backdrop of Georgian Bay.

What You'll Do

- Coach front-line providers in pain/symptom management through case reviews, workshops, and hands-on guidance
- Navigate patients and families through advance care planning and end-of-life support,



and cover the Palliative Nurse Navigator role as needed
- Drive change — lead QI initiatives and help implement the North Simcoe Palliative Care Pathway
- Connect organizations and providers across the region's care network

What You Bring

- RN/RPN in good standing with CNO, with substantive palliative care experience (CAPCE an asset)
- 3+ years in acute/palliative care nursing, with strength in pain/symptom management and system navigation
- A natural coach, with prior coaching or supervisory experience preferred
- QI experience; program development a bonus
- Outstanding communication skills and a genuine commitment to equitable, culturally safe care — including a respectful approach to First Nations, Métis, Inuit, and urban Indigenous self-determined care
- Comfort with PS Suite/Accuro EMRs, openness to learning CloudDx
- Sound judgment and independence under pressure

Required: Vulnerable Sector Search (within 6 months); RNAO membership or professional liability insurance.
