Position Type: Part time

Salary: $29.29 - $32.22/hour

Union: CUPE

Shift Rotation: Days, Evenings, Nights (7am-3pm, 7am-7pm, 3pm-11pm, 11pm-7am, 7pm-7am)

As a Patient Care Assistant, you'll play an essential role in supporting patients and families while working alongside nurses, rehabilitation professionals, and a multidisciplinary healthcare team. About the Transitional Care Unit (TCU)

The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:

- Awaiting long-term care placement
- Working toward increased independence through rehabilitation
- Receiving palliative and comfort-focused care

Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

What You'll Do

As a Patient Care Assistant, you will:

- Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility
- Support patient comfort, safety, and independence
- Assist with nutrition, hydration, and meal support
- Observe and communicate changes in patient condition to nursing staff
- Document care activities accurately and according to policy
- Help maintain a safe, organized, and welcoming patient care environment
- Collaborate with nurses and the interdisciplinary team to deliver exceptional patient-centered care

What You Bring

We are looking for individuals who are:

- Compassionate, dependable, and patient-focused
- Strong communicators who work well within a team setting
- Comfortable working independently while following direction
- Dedicated to providing respectful, high-quality care

Requirements:

- Graduate of a Personal Support Worker program (or equivalent) within the past two years; or one year of full time equivalent experience within the past five years
- Secondary school diploma or equivalent
- Minimum 150 hours of PSW (or equivalent) experience through employment or clinical placement in a long-term care or complex continuing care setting within the past five years
- Ability to communicate effectively in English
- Satisfactory Criminal Record Check and Vulnerable Sector Screening
