The Patient Care Manager is responsible for the overall management of the patient care unit and the quality of care provided. The manager leads and collaborates with the interprofessional team to achieve resident and family centred care. The role includes creating and maintaining a supportive environment for patients, families and staff, providing team leadership, facilitating staff and team development, managing budgets and resources, and initiating and monitoring quality improvement initiatives.
The PCM works with the Veterans Program leadership team to achieve program goals.
The PCM is also accountable for ensuring that care requirements are met, and that patient care issues, problems and concerns identified by patients and their families are addressed. The LTSW unit specialize in the care and quality of life for residential Veterans,
while the LTSE unit is a transitional unit for patients who no longer require acute care, but cannot yet be discharged from the Hospital.
