Green Acres Foundation is seeking an organized, self-motivated Seniors Support Worker who is flexible, has a positive attitude, and has superior customer service skills.

Reporting to the Director of Operations, the Seniors Support Worker is responsible for providing in-depth screenings to identify and connect seniors with appropriate social and health resources.

Key responsibilities and duties:

- Assess client needs
- Create and monitor plans
- Coordinate referrals
- Facilitate system navigation
- Overcome barriers
- Support daily independence
- Collaborate professionally
- Community engagement
- Government reporting
- Data management

Qualifications and Experience

- Bachelor of Social Work is considered an asset.
- Experience in the Human and Health Services field preferably with seniors
- Knowledge of health systems for seniors, including resources to maintain independence




- Knowledge of resources for seniors in rural communities

Required Skills:

- Current satisfactory Criminal Record Check
- Valid Class 5 driver's license
- Current satisfactory driver's abstract
- Current claims experience letter
- First Aid is considered an asset
