As a Clinical Dietitian, you will support adult patients at the Reactivation Care Centre with your clinical expertise. Bringing at least two years of clinical experience, you will play a vital role in patient-centered care, coordinating services, and contributing to quality improvement. Your excellent assessment and leadership skills will ensure optimal patient outcomes during their transition from hospital to home.

Key Responsibilities:



• Conduct comprehensive nutritional assessments for patients • Develop and evaluate personalized care plans • Collaborate with healthcare teams for effective treatment • Transition patients smoothly from hospital to home • Promote quality improvement in nutrition services

Requirements: • Current membership in the College of Dietitians of Ontario • At least 2 years of clinical experience in an acute care setting • BLS Certification required; CNSC preferred • Proficiency with EMR systems and CBORD • Strong communication and organizational skills
