Term: 12 months (maternity/parental leave coverage)

Salary Range: $68,200 - $81,800 annually or $37.47-$44.95 hourly

SUMMARY: As a member of an inter-disciplinary team, the Dietitian provides community-based nutritional care expertise for the prevention and management of diabetes for adults living in Lanark Leeds and Grenville. The dietitian participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and communities affected by diabetes. The dietitian will act as an ambassador for Rideau Community Health Services in the community and promote partnerships with other providers to maximize the capacity of Lanark, Leeds and Grenville to prevent and manage diabetes.

REPRESENTATIVE ACTIVITIES:

· Perform nutrition assessments, develop, and evaluate client-focused nutrition care plans,



perform health education/counselling and other clinical activities according to the College of Dietitians of Ontario standards of practice and the current Diabetes Canada Clinical Practice Guidelines for the Prevention and Management of Diabetes.

· Work closely as a team with the client, RCHS DEP nurses and other members of the health and social services field to provide a holistic, accessible, client-centred diabetes care plan for all clients.

· Make recommendations, individually and in consultation with a Registered Nurse (CDE), to primary care providers on medication and insulin changes for clients when the appropriate knowledge, skill and judgement has been demonstrated (i.e., completion of CDE certification).

· Counsel clients on insulin initiation, adjustments, and pattern management when the appropriate knowledge, skill and judgement has been demonstrated (i.e., completion of CDE certification or demonstrated competency).

· Perform community needs assessments with the Registered Nurse and identify barriers to the prevention and management of diabetes and access to service with an emphasis on food security.

· Plan, implement and evaluate group education activities for clients with diabetes as well as for local health care providers.

· Complete chart documentation using the EMR according to RCHS policies and procedures and College of Dietitians of Ontario standards and participate in chart reviews and case conferences.

· Mentor/supervise dietetic interns or other students as requested.

· Manage diabetes-related compassionate supplies, liaise with drug company sales reps.

· Work in partnership with other agencies to support initiatives related to diabetes prevention and management.

· Provide support at events sponsored by RCHS Diabetes Education Program (DEP).

QUALIFICATIONS:

· Undergraduate degree in food and nutrition from a recognized university or equivalent

· Dietitians of Canada accredited Dietetic Internship or equivalent approved experience or graduate degree in community nutrition.





· Registration with the College of Dietitians of Ontario.

· Three to five years of experience in diabetes education or Certified Diabetes Educator credentials preferred.

· Experience in program development, implementation, monitoring and evaluation.

· Strong health assessment and clinical skills.

· Excellent communication skills and experience working in an interdisciplinary team.

· Strong computer skills, with excellent oral and written communication skills.

· Ability to communicate in both official languages is considered an asset.

· Participate in efforts to enhance capacity through staff development.

· Support and comply with the Occupational Health and Safety policies and procedures.

OTHER

· Ability to work flexible schedule based on program needs.

· Valid Driver’s license and use of a vehicle.

· Canadian criminal record check and a vulnerable sector check that are satisfactory to RCHS.
