We are currently looking to hire a:
Registered Nurse Case Manager – Community Home Care Team1 FTE (35 hours /week) – 1 Year Contract* possible extension*
About the Position
We require one full-time experienced Registered Nurse Case Manager to work collaboratively on the Community Home Care Team. The Community Home Care Team is a growing interdisciplinary team of professionals that supports people within the community, as well as their families, in their homes and other community settings as needed.
RN- Case Manager is responsible for delivering high-quality, client-centered nursing care to individuals living in the community. Working as part of an interdisciplinary community-based integrated care team, the RN will provide clinical assessments, develop individualized care plans, coordinate care with healthcare providers, and support clients in maintaining their independence, health, and quality of life.
Reporting to: Manager, Seniors Support Services
Key Areas of Responsibilities
- Conduct comprehensive nursing assessments in clients' homes and community settings.
- Ability to do Rai-CHA assessments and create care and treatment plans based on client and family goals.
- Develop, implement, and evaluate individualized care plans based on client goals and clinical needs.
- Monitor clients with chronic diseases, acute health concerns, and complex medical conditions.
- Perform nursing procedures within the RN scope of practice, including but not limited to; Wound assessment and management, Medication administration and medication reconciliation, Catheter care, Ostomy care, Vital sign monitoring, Health assessments.
- Ability to work collaboratively with, Personal Support Workers, Physiotherapists, caregivers, and community partners to ensure seamless, coordinated care.
Care Coordination:
- Coordinate services across the interdisciplinary care team.
- Liaise with hospitals, primary care providers, specialists, and community agencies to ensure continuity of care.
- Initiate and lead case conferences.
- Advocate for clients to access appropriate community resources and supports.
Client and Caregiver Education
- Educate clients and caregivers on disease management, medication adherence, nutrition, fall prevention, and healthy living.
- Support clients in developing self-management skills to maintain independence.
- Provide education regarding available community programs and services.
Documentation and Quality Improvement
- Maintain accurate, timely, and confidential documentation in the electronic medical record.
- Ensure documentation meets professional, organizational, and legislative standards.
- Participate in quality improvement initiatives, audits, and program evaluation activities.
- Monitor client outcomes and recommend service improvements.
Qualifications (Knowledge, Experience):
- Bachelor of Science in Nursing (BScN).
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current CPR and Basic Life Support (BLS) certification.
- Minimum 3 years of recent nursing experience.
- Experience in community health, home care, primary care, geriatrics, or chronic disease management preferred.
- Experience working with older adults and individuals with complex care needs.
- Experience with interdisciplinary team-based care is an asset.
- Valid “G” License and access to a vehicle.
Competencies (Skills and Attributes):
- Oral and/or written fluency in other languages would be considered an asset

- Ability to prioritize workload in a fast-paced community setting.
Working Conditions:
- Travel within the community to client homes and partner organizations.
- Flexible work schedule,
including evenings and weekends, is required.
- Valid Ontario driver's licence and access to a reliable vehicle is required.
