- 6-Champlain and surrounding areas, including Ottawa, Kanata, Nepean, Stittsville, Cornwall, Casselman, Hawkesbury, Long Sault
- 7-Waterloo-Wellington and surrounding areas including Kitchener, Cambridge
- 8-Hamilton-Niagara and surrounding areas

Qualifications:

- Masters degree of Occupational Therapy
- Minimum 2 years Experience in field work as an Occupational Therapist
- ADP Authorizer is preferred but not required
- Great communication skills – must be able to effectively communicate and think critically (both written and verbal skills)
- Registration with the College of Occupational Therapists of Ontario (must be in good standing)

Responsibilities:

- Complete community assessments and implement an effective treatment plan for patients (Non MVA files)
- Ensure that all patient’s needs are met
- Collaborate with other team members and health care professionals when needed.
- Documentation-charting to ensure excellent communication in a timely manner
- Identify the need for any beneficial/assistive device for the patient – support with the coordination of the delivery and installation of these devices

Education:

Master’s Degree (required)
