Community Healthcaring KW provides comprehensive primary health care, including medical and dental services, health promotion, illness prevention, and community outreach. Through an interdisciplinary team of health care professionals, we deliver person-centred care that improves health equity, reduces barriers to accessing services, and addresses the social determinants of health to support the well-being of individuals, families, and communities across Waterloo Region.

We are currently recruiting for Registered Practical Nurse to provide services at Broadwalk Family Care. While this position is employed by Community Healthcaring Kitchener-Waterloo, the RPN’s day-to-day work location schedule of hours, as well as applicable policies and procedures, will be determined by the Boardwalk Family Care team to which they are assigned.

Registered Practical Nurse – Boardwalk Family Care

$27.95-$33.68 per hour

FTE 0.8 Part Time, Permanent

30 Hours per week

Position Summary The Registered Practical Nurse (RPN) provides comprehensive, patient-centred primary care and patient support within Boardwalk Family Care. Working to full scope of practice as part of an interdisciplinary team, the RPN delivers direct clinical care including assessments, vital signs, immunizations, chronic disease support, and health education. The RPN collaborates with physicians and other health professionals to improve and support better health outcomes for individuals, and families.

Specific Responsibilities

1. Complete history of clients, anticipate and ensure equipment for client visit is set up in room.
2. Triage client’s needs, either by telephone or in person.
3. Screen and assess clients with minor episodic illnesses according to established protocols.
4. Perform basic nursing skills (e.g., dressing of wounds, injections, suture /clip removal, vital signs — blood pressure, pulse rate, temperature, oxygen saturation — point-of-care testing such as glucose, urine dip and rapid strep, and assisting with other procedures as per office protocols).
5. Provide Health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Practical Nurse and all relevant provincial legislation and regulations.
6. Perform designated controlled acts and delegated procedures under physician-signed medical directives, in accordance with College of Nurses of Ontario practice standards.
7. Perform necessary clinical functions to facilitate delivery of comprehensive primary care and patient flow (e.g., assist providers with prescription ordering; clean, sterilize and reprocess reusable equipment/instruments in accordance with Medical Device Reprocessing standards).

Recognize and respond to clinical emergencies (e.g., anaphylaxis) in accordance with clinic emergency protocols; maintain current BLS/CPR-C certification.
9. Maintain accurate health records and up to date clinical notes in accordance with College of Nurses Standards and policies.
10. Ensure all exam rooms are stocked sufficiently and consistently.
11. Order medical supplies and maintain inventory controls.
12. Maintain contact with pharmaceutical company representatives and maintain a current inventory of pharmaceutical samples.
13. Administer vaccines/immunizations in accordance with Ontario's Vaccine Storage and Handling Guidelines, including monitoring and logging cold chain (fridge temperature) compliance and reporting adverse events following immunization.
14. Order and maintain adequate stock of publicly funded and private vaccines, including monitoring inventory levels, tracking lot numbers and expiry dates, rotating stock (first-expiry, first-out), and reconciling vaccine inventory in accordance with Ontario public health reporting requirements.
15. Organize printed resources such as client education materials, diagnostic and other requisitions.
16. Work during both regular and extended hours (evenings/weekends) as determined by the Health Centre & Boardwalk Family Care.
17. Participate in team and staff meetings and other meetings.
18. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
19. Work in a manner that preserves privacy and confidentiality.
20. Work in a manner that ensures client safety and minimizes risk to clients, volunteers, the Health Centre and Boardwalk Family Care.
21. Work in a manner that demonstrates self-reflection and personal accountability for work performance.
22. Collect, analyze and report on data and relevant information as required.
23. Participate in research as determined by the Health Centre or Boardwalk Family Care.
24. Participate in quality assurance and audits as determined by The Boardwalk Family Care Office
25. Adhere to all Health Centre and Boardwalk Family Care policies, including WHMIS and workplace health and safety training requirements.
26. Adhere to appropriate infection control practices.

Perform any other duties as assigned by your supervisor.
28. Work as part of a team with the Medical Office Administrators (MOAs) to answer phone calls, and book patients. Equally work with the MOAs to help them with the above listed tasks, as appropriate, to ensure the clinic runs smoothly.

Qualifications
1. Current registration as a Registered Practical Nurse in positive standing with the College of Nurses of Ontario.
2. Current BLS/CPR-C certification.
3. A minimum of 1-2 years employment experience, including in a primary care setting. A combination of primary care, community and acute care experience will be considered.

1. Knowledge of available resources accessible to clients (e.g. food banks, local shelters, mental services)
2. Experience working with Vulnerable Population is considered an asset.
3. Demonstrated ability to communicate effectively in English, both written and verbally.
4. Second language is an asset.
5. Competency in use of electronic health records systems is an asset. The Boardwalk Family Care office uses Telus PSS and OceanMD.
6. Ability to work independently as well as an effective member of an interdisciplinary team.
7. Ability to work collaboratively.
8. Ability to maintain strict confidentiality.
9. Experience working sensitively with culturally and economically diverse populations.
10. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
11. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system. This will be the candidate’s responsibility and cost to own and maintain their personal mobile device.
