About the Transitional Care Unit (TCU)

The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:

- Awaiting long-term care placement
- Working toward increased independence through rehabilitation
- Receiving palliative and comfort-focused care

Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

What You'll Do

As a Registered Practical Nurse, you will:

- Assess, plan, implement, and evaluate patient care within the RPN scope of practice
- Deliver evidence-informed nursing care focused on patient goals and outcomes
- Monitor patient progress, identify changes in condition, and collaborate with the healthcare team as needed
- Develop and coordinate individualized care plans
- Support patients and families through rehabilitation, transitional care,



and palliative care journeys
- Delegate, teach, and supervise unregulated care providers
- Participate in discharge planning and care coordination
- Collaborate with physicians, allied health professionals, patients, and families to ensure seamless patient care
- Contribute to quality improvement initiatives that enhance patient experiences and outcomes
- Mentor students and team members while fostering a positive practice environment

Required Qualifications

- Diploma from a Registered Practical Nurse program
- Current registration in good standing with the College of Nurses of Ontario (or eligible for registration)
- Demonstrated knowledge of CNO standards and RPN scope of practice
- Excellent assessment, communication, and organizational skills
- Strong problem-solving and decision-making abilities
- Basic computer proficiency
- Ability to work effectively in a fast-paced healthcare environment
- Proven ability to attend work regularly
- Satisfactory Criminal Record Check and Vulnerable Sector Screening
