We are seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) for our Home Care Team. In this full-time role, you will assess client needs, provide clinical support, and manage care plans effectively. Your contributions help enhance program quality while serving a vital community of over 15,000 individuals each year.
Key Responsibilities: • Conduct client care needs assessments using InterRAI Cha • Provide clinical consultation and support for home care teams • Prepare client assessment reports for health service partners • Participate in program evaluation and quality improvement efforts • Guide the Infection Prevention and Control Committee
Requirements: • Post-secondary nursing degree or diploma • Minimum 3 years’ healthcare experience • Member in valuable standing with College of Nurses Ontario • Valid First Aid & CPR certification • Excellent interpersonal and leadership skills
