In this role, you will support the Hospital Partnership Program, working day shifts to provide essential nursing services for 90-120 days after discharge. You'll perform comprehensive assessments and various nursing procedures, ensuring safe, effective care in collaboration with hospital partners. Your skills in care coordination and monitoring will directly impact patient recovery at home.

Key Responsibilities:



• Assess and monitor client conditions regularly • Develop and coordinate personalized care plans • Ensure high standards of patient care delivery • Document client interactions and changes diligently • Participate in on-call rotations for urgent needs

Requirements: • Registered and in valuable standing with CNO • RPN diploma from a recognized college • Valid First Aid or CPR Certification • Experience in acute care is preferred • Ontario driver’s license and reliable vehicle required
