Are you an experienced RN, Social Worker or Occupational Therapist who still loves working with seniors, but could happily do without the frustrations of a large system?

Keystone Eldercare Solutions Ltd is looking for a thoughtful, experienced Care Manager to join our small, supportive team helping seniors live safely and well at home.

This is not traditional home care. Our Care Managers build trusted relationships with clients and families, notice changes early, solve practical problems, advocate when needed, and help put the right supports in place. We manage and adapt quickly to needs and solve problems on the ground

Many of our clients are living with mild cognitive changes or dementia. Others have transitioned to Assisted Living or residential care, where we continue to provide monitoring, advocacy and family support.

You may be a great fit if you:

- Are an experienced RN, Social Worker or OT, ideally with community, seniors, geriatric or case-management experience
- Enjoy hands-on time with older adults and getting to know the whole person
- Are an excellent observer, communicator and practical problem solver
- Can work independently without needing to be heavily supervised
- Are comfortable communicating with families,



physicians and other health professionals
- Appreciate a respectful, strengths-based approach to dementia care, including the principles of Teepa Snow
- Want meaningful work with a kind, heart-led team where professional judgment is valued

Depending on professional background and scope, the role may include cognitive baseline screening, ongoing health and functional monitoring, appointments, care coordination, family communication and advocacy. Client visits are in homes or facilities in Comox and Courtenay Schedule: Approximately 1–2 days per week, or 3–4 shorter days if preferred. We can build the schedule around the right person.

Compensation: $42–$50/hour as an independent contractor, depending on experience and skilled designation, plus mileage.

This could be an especially good fit for an experienced or recently retired professional who isn’t finished making a difference, but is ready to do it differently.
