Provide expert person-centred care as a Registered Practical Nurse in the D2 unit, specializing in Internal Medicine and Nephrology. Work collaboratively in an interprofessional team to ensure high-quality care in acute settings.
This role requires providing comprehensive care to patients with complex health needs. You'll independently assess and plan care while continuously evaluating interventions and outcomes. The focus is on quality improvement, professional development, and delivering culture-sensitive nursing care in a agile environment.
Key Responsibilities:
• Identify needs of patients and families effectively • Complete assessments and continuously reassess care plans • Implement evidence-based interventions and evaluate outcomes • Commit to quality improvement and reflective practice • Collaborate with RNs to deliver holistic care
Requirements: • Valid registration with the College of Nurses of Ontario • Current CPR certification from the Heart and Stroke Foundation • Preferred RPN diploma with recent acute care experience • Successful completion of the Sunnybrook Advanced Assessment Course • Strong critical-thinking and communication skills
