As a permanent member of our team, you will handle a caseload of 8-10 clients, delivering essential case management services. Your role includes assisting clients with appointments, housing, and financial management while collaborating with a multidisciplinary team for effective care coordination.
Key Responsibilities:
• Provide direct management of complex client cases
