In this position, you will support patients at Hilltop House Long Term Care Facility who are dealing with severe, life-limiting illnesses. Key Responsibilities:



• Deliver nursing care to complex palliative patients • Collaborate in interdisciplinary care team approaches • Assess and manage patient care needs effectively • Facilitate goals of care conversations with patients • Document patient care in accordance with standards

Requirements: • Active RN registration with BCCNM • Two years relevant medical-surgical experience • Palliative care training preferred • Demonstrated assessment skills for patient care • Ability to provide emotional support to families
