Position Title: Social Worker, Caregiver Education and Support

Reports to: Social Work, Clinical Manager

Role Overview

The Social Worker, Caregiver Education and Support position is responsible for offering therapeutic support, education, information, referrals, and resource navigation to people living with dementia and their family caregivers.

The Social Worker, Caregiver Education and Support will also deliver lectures and workshops and participate in other information activities to increase caregiver knowledge of dementia. This role will offer dementia-specific education to people living with dementia, their families, caregivers, and others in Toronto. Utilizing social work processes, they will assist program participants in considering issues relevant to their situation and develop program-solving plans.

The ideal candidate will have minimum 3 years’ relevant work experience, especially in full-time employment in social work, group facilitation or a related field. Working experience with clients with dementia and their families to be considered an asset.

Work location: Main office at 20 Eglinton Ave West, 16th Floor OR the mid-Scarborough Hub at 2660 Eglinton Ave East, depending on client need.

Key Responsibilities - What You’ll Be Doing:

Social Work

- Utilize social work processes during 1-1 interactions to assist individuals – persons with dementia, caregivers with dementia-related concerns and developing problem-solving plans/approaches.
- Facilitate and manage dementia-related support groups
- Work within a team to develop, implement, evaluate, and improve training sessions, support groups, speaking engagements and the counseling process
- Participate in the fulfillment of the Society’s mandate, including maintaining records, statistics, and assisting with program development
- Support/supervise social work field placement students, as needed.

Caregiver Education and Support

- Develop, deliver and facilitate dementia information sessions and educational workshops to various stakeholders across the city, focusing primarily on people living with dementia, and caregivers, and occasionally, as needed, with the general public
- Coordinate education events and speaking arrangements for caregivers and people living with dementia.
- Provide support to clients and facilitators participating in the Peer Support Group program.




- Utilize social work processes to assist program participants in considering issues relevant to their situation and developing problem-solving plans
- Promote awareness and understanding of the importance of dementia specific education and information to diverse groups
- Maintain ongoing contact with colleagues and contacts external to the organization clients to ensure educational requirements are met.
- Act as a resource for inquiries from the general public and for the Society’s programs and services.
- Work within a team to develop, implement, evaluate and improve workshop content, process, and speaking engagements to ensure that they have met the requirements of the target group.
- Keep up to date with new developments in dementia research and care as well as relevant services in the community.
- Acting as a staff liaison to volunteer committees, as assigned
- Liaise with the community to increase awareness of the Society’s mandate and services

Expectations - What You Bring:

Education and Experience

- University degree in Social Work. Registration with the Ontario College of Social Workers and Social Service Workers, in positive standing, is required.

- Minimum 3 years’ relevant experience:

- - In full-time employment in social work, group facilitation or a related field
- Work experience with clients with dementia and their families is an asset.

Other Knowledge, Skills, Abilities or Certifications:
- In-depth knowledge of Alzheimer’s disease and other dementias, community healthcare systems, and available resources
- Experience developing and facilitating educational, and group programs
- Excellent presentation, public speaking, facilitation, and group work skills
- Proficiency in Microsoft Office and general computer applications
- Demonstrated commitment to professional development is essential.
- Outstanding inter-personal, organizational, and teamwork skills to accomplish goals with the ability to work independently and take initiative
- High level of attention to detail, professionalism, and strong work ethics
- Bilingualism (English and a second language) and access to a vehicle are considered assets.

Employment Conditions

- Work status: Must be legally entitled to work in Canada.



Experience gained in Canada or internationally is valued.
- Travel requirements: Some travel to partner sites to facilitate workshops and groups. This role will be required to work out of our East and Central office locations, with a requirement of minimum one day a week at either office location.

- Physical demands: Significant periods sitting at the computer, but with the opportunity to move away from the workstation. Sensory attention is required for the majority of the workday (looking at computer screens, reading documents, etc.)

Total Compensation / What We Offer:
- Hiring salary range: $64,000 - $70,000.
- Benefits: Comprehensive group benefits after three months and pension plan with up to 5% employer matching after 1 year of service.
- Paid time off: Paid Vacation (15 days), sick leave (18 days), personal days (2) & Equity Floater days (3) per fiscal year (prorated from start date)
- Work arrangement: Hybrid Work; working from home, choice of offices and flexible hours, minimum of 1 day per week.
