The Fracture Clinic is an outpatient clinic serving the needs of a diverse adult population of patients requiring the services of orthopaedic surgery.
As a key member of the interdisciplinary team, the Social Worker will provide services to adult outpatients in the Orthopedic Fracture Clinic. The services provided range from psycho-social assessment and treatment; crisis intervention; advocacy; supportive counseling; consultation, patient/family/staff education; linking with community resources.
The Social
Worker will enjoy a challenging practice, which requires excellent clinical skills, flexibility and a desire to work both independently and collaboratively in a variety of settings.
This is a temporary, study-based position created to support a pilot trial of embedded social work services in the outpatient fracture clinic.
- Masters degree in Social Work from an accredited University
- Certification as a Social Worker with the Ontario College of Social Workers and Social Service Workers
- Minimum of 3 to 5 years of clinical social work practice involving individual and family counseling and therapy
- Previous experience in the provision of psycho-educational or therapy groups
- Demonstrated expertise in interviewing, the completion of comprehensive psychosocial assessments and the development of a plan of action
- Excellent communication, interpersonal, teamwork and crisis intervention skills
- Demonstrated skill and ability in provision of health professional consultation and education
- Demonstrated ability to identify and work with appropriate resources to address patient and family needs
- Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act and Child and Family Services Act
