Make a meaningful impact as a Full-Time Rehabilitation Services Coordinator with March of Dimes Canada in Simcoe County. Support clients with disabilities through tailored rehabilitation plans in a compassionate setting.

In this 12-month contract role, you will utilize your expertise to create and implement Individual Rehabilitation Plans to meet client goals. Collaborate with Behaviour Consultants and community support supervisors to achieve service deliverables and provide oversight to a team of rehabilitation workers. Daily travel within Simcoe County is required, with mileage reimbursement offered.

Key Responsibilities:
• Develop and track Individual Rehabilitation Plans for clients
• Collaborate with supervisors and Behaviour Consultants




• Complete progress reports and incident documentation
• Coordinate specific services both at home and in the community
• Assess client skills and follow up on their development

Requirements:
• Preferred post-secondary education in Human and Social Services
• Minimum of three years’ experience in Human Services
• Current CPR/First Aid certification preferred
• Proficiency in Microsoft Office applications
• Strong organizational and communication skills
