Deliver essential nursing care to seniors as a Licensed Practical Nurse at Extendicare. This part time role focuses on patient-oriented, goal-directed care in a supportive environment.
As a Registered Practical Nurse at Extendicare, you'll collaborate with an inter-professional team, enhancing your skills in gerontology and rehabilitation. Your responsibilities include conducting clinical assessments, managing care plans, and providing direct care and medication administration. You'll also facilitate resident education and handle care coordination effectively.
Key Responsibilities:
• Participate in inter-professional team meetings for care planning
• Conduct and document resident clinical care assessments
• Implement and evaluate individualized care plans
• Administer medications and maintain safety standards
• Address resident concerns and communicate effectively
Requirements:
• Current registration as a Licensed Practical Nurse
• Experience in senior’s care environments preferred
• Strong communication and critical thinking skills
• Basic Cardiac Life Support certification required
• Willingness to obtain a Vulnerable Sector Check
