In this key managerial role, the Supported Housing Program Manager drives the strategic and operational success of care delivery to meet resident needs. Collaborating within an interdisciplinary team, you'll coordinate comprehensive care planning while providing leadership to nursing and support staff. Your commitment to advocating for residents ensures they receive the best care possible within our mission-driven organization.

Key Responsibilities:
• Direct care services to achieve quality resident outcomes
• Collaborate on complex case navigation with the interdisciplinary team




• Evaluate care service performance and implement quality improvements
• Develop and execute clinical care plans based on evidence-based practices
• Ensure compliance with relevant standards and regulations

Requirements:
• Regulated Health Care Qualified Qualification (LPN, RN, OT or RPN)
• 3-5 years of relevant supportive living experience
• Knowledge of the Continuing Care Health Services Standards
• Effective communication and interpersonal skills
• Current BLS certification required
