AgeCare seeks a Licensed Practical Nurse (LPN) to deliver goal-directed nursing care in a person-centered setting. Reporting to the Director of Care, you will work with an interdisciplinary team to support residents’ dignity and well-being in rural and select urban communities.
You will assess residents, develop and update care plans, administer medications, educate families, coordinate with physicians, and ensure compliance with policies.
