Key Responsibilities:
• Provide evidence-based, in-home patient care

Requirements:
• Active RN/NP or Physician Assistant license
• Minimum two years of direct patient care experience
• BLS certification, DEA eligibility, and EHR experience
• Strong decision-making and critical thinking skills
• Background in managed care or value-based care preferred
