Step into a pivotal role as Clinical Lead in Home and Community Care with Six Nations. This full time position emphasizes culturally aligned clinical services and trauma-informed practices.
You will report directly to the Senior Manager of Primary and Clinical Care, steering the clinical guidance and support for the Home and Community Care (HCC) Team. Your expertise will integrate Western medical practices with Traditional Hodi:noshoni:/Rotinonhsion knowledge, ensuring quality care across the community.
Key Responsibilities:
• Oversee adherence to health regulations and standards • Encourage inter-professional collaboration among staff • Assist with recent member onboarding and training • Develop client care plans in collaboration with HCC Managers • Engage in quality improvement projects and committee participation
Requirements: • Master’s or Bachelor’s in a regulated health profession • Active or eligible registration with Ontario Professional College • Proven leadership in an interdisciplinary setting • Solid understanding of cultural healthcare principles • Proficiency with EMR and documentation standards
