Key Responsibilities:
• Provide comprehensive nephrology care across clinical settings
• Assess patients and develop effective care strategies




• Conduct dialysis rounds and collaborate with healthcare teams
• Document all patient interactions in Epic software
• Assist with chronic disease management and patient education

Requirements:
• Completed Master’s in Physician Assistant or Nurse Practitioner
• Illinois state licensure or eligibility
• Experience in nephrology (1-2 years preferred)
• Passionate about improving healthcare delivery
• Strong collaboration and communication abilities
