ABOUT THE ROLE Reporting to the Director of Care, is responsible for the organization and management of the Nursing Program at the Long-Term Care Home(s) (the Home(s)) including responsibilities for effective day to day operation of nursing services in accordance with the organizational values, the Residents’ Bill of Rights, applicable legislation and regulations, corporate policies and procedures, and the collective agreements, ensuring safe and effective medical care is provided in the Homes and ensures the maintenance of a safe environment for residents, staff and visitors, mentoring and development of staff; budget development, and continuous quality improvement measures within a resident-centred multidisciplinary environment that promotes independence and quality of life for residents.
WHAT YOU'LL BE DOING Supports the Director of Care in establishing and implementing nursing policies, procedures and programs to ensure that the level of nursing care conforms to applicable legislative, regulatory and professional standards; demonstrates and apply current knowledge of best practices in the provision of care in long-term care.
Provides subject matter expert consulting advice to the organization on all matters of a clinical resident care nature, and support the Homes’ infection control program, education program, and health and safety program.
Formulates and assigns the work of the staff and ensures appropriate nursing staffing levels to meet the needs of the residents.
Coordinates scheduling with other management and supervisory personnel both on an informal and formal basis.
Coordinates multi-disciplinary teams in the development and implementation of individual care plans for all assigned residents of the Homes.
Assists in the development of the nursing unit objectives; communicates plan to the staff and ensures staff meet expectations.
Consults with the care team members regarding resident care, as required,
and coordinates the established system of resident assessment and classification to ensure that the individual care needs of all assigned residents are met and that staffing patterns are appraised.
Ensures adherence to health, safety and security requirements by staff.
Promotes and ensures effective communication within the Home and encourages harmonious interpersonal relations between staff, residents, family members, visitors, volunteers and external service providers.
Maintains critical awareness of evolving community and seniors issues by participating in long term care planning activities.
Ensures proper supplies of all required material and equipment and provides advice and assistance to the Director of Care, regarding planning and control measures to meet needs.
Assists the Director of Care in formulating Nursing policies and procedures.
Demonstrates and applies current knowledge of continuous quality improvements in long-term care environment.
Responsible for all matters related to the continuous quality care improvement, all required programs under the Long-Term Care Homes Act and Ontario Regulation 79/10.
Initiates and implements such procedural changes, as may be required, to maintain or improve the quality of nursing care and efficiency of staff, in compliance with established budget.
Implements quality control measures to ensure that medications are safely and securely stored and properly distributed and administered to individual residents.
Continually monitors and measures the quality of long-term care services to ensure that the care delivery and performance and nursing practices of staff are in accordance with the standards of the Ministry of Health and Long-Term Care, the Department and the College of Nurses.
Maintains a continuous awareness of the state-of-the-art developments in nursing administration and long-term care.
Supports the Director of Care in Quality Assurance and Continuous Quality Improvement activities to ensure a strong clinical care quality assurance system of internal controls and an setting of continuous quality improvement exists within all functional areas within the scope and duties of your role.
Maintains a continuous awareness of current and emerging developments, best practices, innovation and emerging issues in long term care nursing.
Assists the Director of Care in developing annual business/work plans and in developing service plans and staffing proposals.
Provides input into budget and business plan development, policies and procedures.
Provides input into the development of policies and procedures.
Assists with management of team budget/expense management.
Ensures files and records are properly maintained and conducts analysis of integrated program statistics, as required.
Tracks and monitors referrals to other health care facilities and services and maintains records.
Supervises staff, including participation in recruitment, selection, hiring, scheduling, assigning and monitoring work, determining training and development needs, coaching and mentoring, conducting performance appraisals and determining/recommending disciplinary action up to and including dismissal in accordance with collective agreements, Regional policies and practices.
Ensures that operating staff work in a safe manner and utilize all required health and safety equipment and protective devices and follow all measures and procedures as required by the Occupational Health and Safety Act and Regulations and Regional Policies.
Responds to customer enquiries directly and/or resolves difficult or highly sensitive complaints either verbally or in writing.
Ensures that complaints are handled in compliance with applicable policy and legislation.
Liaises,
fosters and maintains positive relationships with internal staff and external contacts, government and non-government agencies and the public.
Implements customer service strategies to ensure that services provided meet Regional customer service standards.
Provides input to or prepares reports, briefing notes, presentations, statistics and analysis.
Participates on committees, meetings, task forces, work groups and special projects, as directed.
Assumes functions of the Director of Care in that person’s absence, as directed.
Perform other duties as required to meet Divisional/Branch and Departmental objectives.
Assumes on-call duty after regular working hours, weekends and statutory holidays on a rotating basis.
May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan.
WHAT WE'RE LOOKING FOR Successful completion of a Diploma in Health Sciences or related field from an accredited College.
Current Registered Nurse Certificate of Registration from the College of Nurses of Ontario with current good standing as a Registered Nurse with the College of Nurses of Ontario.
Minimum three (3) years’ experience in a nursing program service delivery/health care sector including experience in long term care, and demonstrated supervisory or leadership experience.
Valid Ontario Class “G” driver’s license and reliable vehicles for use on corporate business.
As a condition of employment, candidates will be required to provide a satisfactory Police Vulnerable Sector Check (PVSC), tuberculosis screening and proof of immunization in accordance with the Long-Term Care Homes Act (O.Reg.79/10).
Sound knowledge of relevant best practices and research related to gerontological nursing, Regulatory Health Profession Act and adult learning principles. Advanced knowledge and skills in resident assessment, critical thinking, problem solving and clinical decision making.
In-depth knowledge of RAI-MDS. Knowledge of Lean process and/or other Continuous Quality Improvement and Efficiency Focused methodologies.
Demonstrated knowledge of relevant Standards, Acts and Regulations pertaining to nursing care and long-term care facilities, including best practices in the Long-Term Care sector, CQI and Risk Management principles.
Leadership competencies including operating strategically, leveraging diversity and differences, customer focused, ensuring accountability, managing complex problems, financial management, employing interactive and effective communication, fostering collaborative relationships, cultivating engagement to drive vision and purpose, managing and developing talent, establishing and maintaining trust, displaying self-awareness, and demonstrating resiliency.
Ability to travel to offsite locations, as required.
