Key Responsibilities:
• Provide comprehensive, evidence-based primary healthcare
• Facilitate chronic disease management and wellness initiatives




• Collaborate in interprofessional case conferences
• Mentor medical learners and contribute to their education
• Take part in quality improvement efforts

Requirements:
• Independent practice license from CPSO
• CCFP certification or equivalent experience
• Commitment to diversity and inclusion in healthcare
• Interest in community health and preventative approaches
• Recent Vulnerable Sector Police Check
