Based at the Natsékhi Kų̀ Health Centre, this position involves close collaboration with community members and healthcare professionals. Your expertise in counseling and crisis intervention will aid in navigating complex health needs, providing crucial support to those facing challenges in accessing care.

Key Responsibilities:
• Support clients in accessing community health services
• Advocate for their rights and health needs
• Coordinate comprehensive wellness plans
• Educate clients and families to improve health literacy




• Work collaboratively with other stakeholders for integrated care

Requirements:
• Bachelor of Social Work with 2+ years of experience
• Skills in motivational interviewing and psycho-social assessment
• Understanding of trauma and family systems
• Excellent communication and judgment skills
• Valid Driver's License and willingness to obtain First Aid certification
