Evolve Therapy Services is looking for a Director of Rehabilitation in Franklin, MN! Lead therapy operations and provide quality patient care in a skilled nursing setting.

As the Director of Rehabilitation, your role encompasses overseeing the therapy program and ensuring compliance with all regulations. You’ll collaborate with interdisciplinary teams to implement effective procedures and support therapy staff with ongoing training. Your leadership will enhance the quality of services provided to patients.

Key Responsibilities:
• Lead therapy program management at facilities
• Train and oversee therapy staff
• Ensure compliance with regulatory requirements
• Communicate professionally with patients and families




• Complete other responsibilities as assigned

Requirements:
• Bachelor’s or higher in Physical, Occupational, or Speech Therapy
• Active licensure (PT, PTA, OT, OTA, SLP) in practice state
• Experience with PDPM and MDS management preferred
• Strong verbal and written communication skills
• Commitment to ongoing education and license maintenance
