Enhance diabetes care as a Registered Nurse at WellFort Community Health Services, located in Brampton, ON. This full time role focuses on education and support for clients with diabetes.
As a key member of the Diabetes Education Program, you will educate and counsel adults with Type 2 diabetes, helping them achieve optimal health through personalized care and support. Your work will involve assessing clients, developing care plans, and collaborating with healthcare professionals to ensure comprehensive diabetes management.
Key Responsibilities:
• Conduct detailed diabetes assessments and follow-ups
• Design and lead impactful group education sessions
• Provide individual counseling and self-management support
• Partner with community agencies for outreach
• Document clinical interactions proficiently
Requirements:
• Bachelor of Science in Nursing (BScN) preferred
• Certification as a Diabetes Educator is required
• Active CPR certification
• Minimum three years of recent nursing experience
• Prefer experience in community health education
