St. Leonard’s Community Services is seeking a Community Case Worker for its Reintegration Program on an 18-month contract. Focus on case management and support for individuals facing reintegration challenges.

In this rewarding role, you will be responsible for developing support plans and managing daily operations for community-referred clients. You will assess clinical suitability and connect participants to necessary services. Robust interpersonal skills are vital as you will liaise with community resources and provide crisis intervention.

Key Responsibilities:
• Develop and implement tailored support plans for participants
• Network and build strategic community partnerships
• Assess risk and suitability for various participants




• Collaborate with other professionals and stakeholders
• Provide advocacy during crisis management

Requirements:
• Post-secondary education from an accredited institution
• 1-3 years relevant social services experience
• Intermediate First Aid/CPR certification
• Valid G Class Driver’s License and reliable vehicle
• Candidates proficient in both official languages will be preferred
