with the applicableregulatory body:College ofNurses of OntarioCollegeof Physiotherapists of OntarioCollegeof Occupational Therapists of OntarioOntarioCollege of Social Workers and Social Service WorkersRecentexperience in community health or a related fieldKnowledgeof the health care delivery system and community resourcesExcellentinterpersonal, communication, assessment, problem-solving anddecision-making skillsEffectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environmentEstablishedability to accurately complete required documentation, reports and formsAvalid driver’s license and access to a reliable vehicleAdvanceoral and written proficiency in English and French is essentialWehave a mandatory COVID-19 vaccination policy.
