Become a vital part of the Upper Grand Family Health Team as a Community Kinesiologist in a permanent 0.8 FTE capacity, promoting health in the Fergus, Elora, and Arthur areas.

This role involves a mix of clinical responsibilities, including chronic disease management, cardiac rehabilitation, and exercise support. Collaborate with a dedicated healthcare team to execute various health promotion strategies tailored to the unique needs of individuals and families in your community.

Key Responsibilities:
• Provide health support and programming for chronic disease
• Develop and evaluate health promotion and prevention strategies
• Conduct in-home visits and utilize virtual care technologies




• Maintain current knowledge in evidence-based practices

Requirements:
• Bachelor's or Master's degree in a relevant health field
• Valid membership in a regulatory association
• Familiarity with chronic pain and mental health strategies
• Solid communication skills and computer proficiency
• Valid Ontario Driver's License and access to transportation
