This permanent, full time role calls for a Manager dedicated to fostering cohesive teamwork in delivering health services. You will be responsible for leading initiatives that integrate clinical, wellness, and educational services while promoting quality care for various priority populations. Reporting to the VP, Integrated Care and Experience, you will ensure the effective implementation of strategic initiatives aligned with DCHC’s goals.

Key Responsibilities:
• Support strategic planning and operational needs
• Oversee the performance of diverse interprofessional teams




• Promote quality improvement methodologies and practices
• Engage with community partners for integrated care solutions
• Facilitate compliance with health standards and regulations

Requirements:
• Health care or related undergraduate degree
• Minimum 5 years in community health management
• Proven expertise in integrated care models
• Strong organizational and leadership capabilities
• Familiarity with quality improvement tools and frameworks
