As a Case Manager, you will play a key role in assisting women dealing with homelessness and mental health issues. Your responsibilities will include developing personalized case plans, conducting assessments, and ensuring clients receive the support they need to thrive. Collaboration with community partners is essential in this impactful role.
Key Responsibilities:
• Build strong, trusting relationships with clients
• Conduct detailed assessments to identify strengths and needs
• Regularly update individualized case plans with clients
• Facilitate safety planning and risk assessments
• Provide crisis intervention and supportive counseling as needed
Requirements:
• Bachelor’s degree in social work or related field
• Minimum 2 years of relevant experience
• Certification in CPR and First Aid required
• Must be registered with a skilled association
• Strong knowledge of addictions and recovery principles
