Join Ontario Health atHome as a Permanent Full time Nurse Practitioner in Palliative Care, focused on patient-centered treatment. Contribute to a collaborative team delivering essential healthcare services.

This role is integral to providing care for patients unable to access standard services. As a Nurse Practitioner, you will evaluate complex health concerns and manage chronic pain within the community framework. Your expertise will ensure quality and safety throughout the patient’s palliative journey, and you may also work from various Hospice sites in the Erie St. Clair Region.

Key Responsibilities:
• Offer comprehensive Nurse Practitioner services for palliative patients
• Assess and provide consultative guidance for symptom management
• Collaborate with Hospice staff on patient care




• Maintain high standards in evaluating complex health situations
• Travel to different Ontario Health atHome sites as required

Requirements:
• Currently registered Nurse Practitioner with the College of Nurses
• Thorough understanding of community health services
• Experience in palliative care education and management
• Ontario Driver's License and vehicle access needed
• Compliance with mandatory COVID-19 vaccination
