Position Summary:
Inclusion Northumberland is a non-profit organization that provides support and services to people with developmental disabilities that promotes opportunities for personal growth within their community. We are a Nationally Accredited organization and have maintained our Accreditation standing since 2000 in each of our quality reviews facilitated by the Council on Quality Leadership.
This is a challenging position for a person with confidence in their organizational, data entry, administrative and communication skills. The successful candidate will be responsible to coordinate support and services within supported group living locations and community based settings. The successful candidate will be able to demonstrate a strong understanding and knowledge of: Developmental Services sector in Ontario, scheduling systems and software, mentor and orientation of new employees, Passport/Fee for Service Funding, Personal Outcome Measures, Core Competencies, and all Legislation and Regulations as they relate to; Supports & Services, the ESA, OHS, Quality Assurance Measures, our Collective Agreement, Controlled Acts, Fire Code and Health Services Regulations.
Summary of Responsibilities (including but not limited to):
Ensure that all support to people are respectful of their dignity, rights and well-being at all times

Provide ongoing communication, support, resources and training for employees on site through day-to-day guidance and support (evening/weekend hours will be required)

Assist with ensuring that all required supports, support plans, funding application and Personal Outcomes have been identified, facilitated and implemented for each person in accordance with their personal preferences

Assist the Supervisor in holding employees accountable to the expected standard of quality services and support delivery

Problem solve to effectively and collaboratively handle issues and conflict

Liaise with families, employees, Supervisors and other service providers in the annual updates of support plans and protocols (i.e. Health, PRN Protocols, Employee Safety, Safety Monitoring, Best Practices for Incident Prevention, Behaviour Support Plans, financial agreements)





Assist with orientation and mentoring of new employees as it relates to understanding quality supports, duties of the position and applicable controlled acts, including theory, practical, formal monitoring

Review all incoming incident reports to provide input and training on corrections to be made and alert appropriate person, if immediate review is needed

Assist with coordinating team meetings, team training needs and staff development opportunities

Review and approve the monthly LifeShare and Supported Independent Living Health & Safety Checklists submitted by Direct Support Professionals and communicate issues or concerns to the Supervisor

Conduct weekly reviews of MARS and doctor’s order at Supported Group Living locations to ensure policy compliance and identifying trends and/or concerns to the Supervisor

Conduct monthly location audits at Supported Group Living locations to ensure compliance with health and safety standards and compliance regulations, while promoting a positive and healthy living/work environment at all locations

Conduct and maintain records of random weekly financial monitoring in accordance with Policy PRS-10 People’s Finances

Assist in Ministry Compliance reviews and ensure program compliance through observing the completion of six month compliance checklists with Primary Supports, while ensuring information is updated on AIMS, Individual Support Plan is accurate and any new information is shared with the team and the Supervisor

Serve as a backup for scheduling in the absence of the Supervisor or during an unexpected employee leave of absence

Act as the location management representative in the Supervisor’s absence

Manage the processing of Employee Mileage and Expense Forms

Identify organizational trends/challenges and assist in developing solutions for resolve or improvement

Assist with ensuring the completion of all necessary administrative and service delivery documentation as required by MCCSS and the Organizations Policies and Procedures

Provide support during Ministry Compliance reviews and actively contribute to identifying areas of improvement and implementing corrective actions to maintain continuous compliance with MCCSS

Actively contribute as a positive and professional member of the Inclusion Northumberland Management Team

Ensure an orderly, safe and clean working environment at all locations

Work with both internal and external committees, as required

Participate in emergency On Call rotation as scheduled





Identify and advocate for change when barriers to quality service delivery are identified

Other duties as assigned

Education and Qualifications (including but not limited to):
Ideal candidates will have a degree/diploma from a recognized university or college in a related human services discipline or the skills and work experience diverse enough to be considered equivalent. The candidate will also have significant knowledge of/experience with: Supported Group Living, Community Participation Services, Individualized Funding, Life Share, Employment, Respite and Supported Independent Living supports as well as:
A minimum of 5 years experience working with people with developmental disabilities

A minimum of 3-5 years of coordinating support and services within the Developmental Services Sector, preferably in a Supportive Group Living and or Community/Family support role

Demonstrated experience in coordinating multiple employees, locations and support protocols/plans for people supported

Strong communication abilities both written and oral

Strong facilitation/training/presentation skills to groups small and large

Exceptional interpersonal, organizational and problem solving skills

Proficiency with information technology, including Microsoft Office 365, Inclusion Systems and AIMS

Ability to maintain a skilled attitude; ability to handle stressful situations

Good standing with above average attendance record, proven history of above average performance and a demonstrated commitment to Inclusion Northumberland’s Mission, Vision, Values and Principles

Demonstrated experience conveying a helpful, supportive, positive and confident attitude

Ability to maintain confidentiality and conduct work within appropriate privacy standards

Demonstrated initiative, creativity, forward thinking and team spirit

Ability to manage multiple tasks and competing priorities

Ability to work both independently and as part of a team

Experience working with diverse populations

Demonstrated leadership and mentoring skills

Must have a reliable vehicle for the purpose of work, a valid Ontario Class G Driver’s license, an acceptable Driver’s Abstract and automobile insurance coverage of at least $1 million for third party liability

Must have valid Standard First Aid – Level C CPR and NVCI Certification
