Exciting chance for an organized Patient Care Coordinator at Kootenay Boundary Regional Hospital in Trail, B.C. This relief position works rotating days from 06:30 to 17:40.
As a Patient Care Coordinator, you'll oversee clinical operations while ensuring the delivery of quality care guided by BCCNM standards. This role includes supervising staff, assisting with clinical decision-making, and providing education to families. With a focus on patient transitions, you will facilitate resources and promote a supportive environment for both patients and staff.
Key Responsibilities:
• Manage day-to-day operations in the clinical area
• Support staff supervision and clinical decisions
• Coordinate patient movements through the healthcare system
• Collaborate with the interdisciplinary team on resource needs
• Analyze and recommend improvements for patient care standards
Requirements:
• RN with relevant experience in patient coordination
• Effective organizational and supervisory abilities
• Knowledge of patient-centered care principles
• Strong communication and team collaboration skills
• Drive for quality improvement in healthcare delivery
