Evolve Therapy Services invites compassionate Occupational Therapists to provide care in a skilled nursing facility in Olivia, MN. Full-time, part-time, and PRN options are available, along with a sign-on bonus and tuition reimbursement.

As an Occupational Therapist, you will lead the planning and execution of therapy services, ensuring quality resident care. Responsibilities include screening residents, developing and implementing treatment plans, and working closely with healthcare teams. Your commitment to excellence will enhance patient outcomes while following state and federal regulations.

Key Responsibilities:
• Screen residents to identify optimal treatment interventions
• Evaluate performance and create individualized therapy plans




• Communicate findings to patients and their support networks
• Schedule therapy sessions in collaboration with team members
• Ensure all documentation and billing are accurate

Requirements:
• Degree in Occupational Therapy is required
• Active Occupational Therapy license in the state of practice
• Ongoing career development for certification upkeep
• Strong interpersonal and communication skills
• Experience in a skilled nursing environment is advantageous
