Service Navigation Coordinator – Toronto (Full-Time)
The Organization Spinal Cord Injury Ontario is an expert in the field of spinal cord injury (SCI) community services. Founded in 1945, SCI Ontario’s mission is to “provide peer experiences, education, advocacy, and ongoing services for those impacted by spinal cord injuries.”. Job Summary As part of a dynamic provincial organization, the Service Navigation Coordinator provides coordination of services, active support, resources, and education. The position focuses on client centered goals and solutions to navigate system barriers that prevent persons with spinal cord injuries living the life they choose.
This is an existing position and an active vacancy. The Service Navigation Coordinator position requires an innovative and resourceful individual to assist with serving clients in Toronto area. This full-time position is based out of the Toronto provincial office with travel throughout the community. The current work arrangement includes approximately three (3) days per week working on site at the office, with the remaining workdays varying based on client, community, and operational needs. These days may include community/client visits or remote administrative work, depending on the schedule and priorities for the week. Regular travel throughout Toronto is required.
Reporting to the Manager, Service Navigation & Intake, you will: Work with individuals with a spinal cord injury and their families in acute care, rehabilitation and in the community
Act as a resource, navigator, advisor and facilitator, supporting and advocating for clients, their family and the community
Work with clients on goal setting and attainment in the areas of equipment, housing,



transportation, health and wellness, recreation, personal support services, and funding.
Provide information and education on a wide variety of topics related to spinal cord injury
Qualifications: University Degree in rehabilitation service, social work, social sciences or equivalent education and experience
Minimum three years’ experience in a rehabilitation setting within community services as well as other social and health related disciplines or similar related experience
Holistic/generalist knowledge of mobility disabilities and their functional and emotional implications
Proven assessment skills, conflict resolution and mediation experience
Excellent verbal and written communication skills
Solid planning and time management skills
Comprehensive knowledge of community service and support networks
Experience with case management databases; and knowledge of interRAI CHA, an asset
Demonstrated computer proficiency working in an MS Office environment
Ability to travel within the region, with occasional provincial travel.
Bilingual, written and verbal communication in French, an asset
