We are seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) for our Home Care Team. In this full time role, you will assess client needs, provide clinical support, and manage care plans effectively. Your contributions help enhance program quality while serving a vital community of over 15,000 individuals each year.
Key Responsibilities:
• Conduct client care needs assessments using InterRAI Cha
• Provide clinical consultation and support for home care teams
• Prepare client assessment reports for health service partners




• Participate in program evaluation and quality improvement efforts
• Guide the Infection Prevention and Control Committee
Requirements:
• Post-secondary nursing degree or diploma
• Minimum 3 years’ healthcare experience
• Member in good standing with College of Nurses Ontario
• Valid First Aid & CPR certification
• Excellent interpersonal and leadership skills
