Mount Carmel Clinic seeks a Community Health Facilitator to coordinate intake, assess client needs, and connect them with relevant team resources. You will work under the Program Manager - ACT and ensure access to services for clients across the Mount Carmel Clinic network.
The role requires a post-secondary health or social services degree or diploma, at least five years of client service experience, and a valid driver's license.
