Key Responsibilities:
• Act as a liaison for CareRx Pharmacy and community homes
• Travel to meet clients in LTC and retirement settings
• Implement medication reviews and documentation processes
• Provide health education and training workshops
• Support quality assurance through medication audits
Requirements:
• Valid registration with Ontario's College of Pharmacists
• Experience in fostering healthcare collaborations
• Robust organizational and prioritization skills
• Familiarity with pharmacy practice standards
• Possess a current driver's license
