Elevate resident care as a Wellness Nurse at The Neighborhood at Priceville, working overnight shifts to ensure health and wellbeing. Provide essential support and medication management.

In this LPN role reporting to the Wellness Director, you will monitor residents’ needs through assessments and manage their Personalized Service Plans. Your leadership will foster a quality care environment, addressing medication administration, family communications, and service quality, enhancing the residents' overall wellness journey.

Key Responsibilities:
• Identify and assess resident needs and services
• Ensure documentation reflects any clinical care changes
• Aid with clinical sections of monthly wellness updates




• Notify leadership of medication or service changes
• Assist with vital signs and resident care as necessary

Requirements:
• Must hold a Licensed Practical Nurse or Registered Nurse credential
• At least one year experience with elderly or disabled individuals
• Strong leadership capabilities needed
• Familiarity with Assisted Living and Dementia care preferred
• Flexible schedule including evenings and weekends required
