In this vital role, you will be responsible for establishing effective care plans tailored to resident needs and ensuring their continuous evaluation. You will directly supervise Health Care Aides, leading staff in training and development initiatives. Engage in strategic planning with the leadership team and act as a liaison with community agencies to enhance service delivery.
Key Responsibilities:
• Create and update personalized care plans for residents
• Host resident care conferences with families
• Supervise, train, and mentor Health Care Aides
• Contribute to strategic site leadership initiatives
• Ensure compliance with medication administration
Requirements:
• At least 3 years in a geriatric care environment
• Valid nursing program certification with proficiency
• Strong ability to mentor and develop clinical staff
• Knowledge in Continuous Quality Improvement practices
• Willingness to work varied shifts and on-call
