Responsibilities will involve conducting assessments, implementing care plans, and administering medications, all while coordinating with multidisciplinary teams to optimize resident care and support.
Key Responsibilities:
• Collaborate in inter-skilled care teams effectively
• Manage clinical care assessments and documentation
• Evaluate the effectiveness of individualized care plans
• Provide education and support to residents and families
• Coordinate emergency responses and medication safety
Requirements:
• Current registration as a Licensed Practical Nurse
• Preferred experience in senior care environments
• Strong clinical knowledge and critical thinking skills
• Valid Basic Cardiac Life Support certification
• Willing to obtain a Vulnerable Sector Check
