As part of our dedicated MHSU team, you will provide critical services focused on rehabilitation and harm reduction for those struggling with complex challenges. Your responsibilities will include crisis management, thorough risk assessments, and coordinating individualized care plans with a focus on client-centered approaches.

Key Responsibilities:
• Execute risk assessments and crisis intervention plans
• Engage in effective outreach and case coordination
• Collaborate to develop tailored care strategies




• Maintain documentation and deliver client advocacy
• Promote community integration and empowerment

Requirements:
• Background in mental health or substance use care
• Strong documentation and communication skills
