The Windsor Essex Gets You Home (WEGHO) & Advocacy Client Service Navigator (CSN) supports clients as they transition home from hospital, access community services, and resolve service-related concerns. In this role, you will coordinate referrals and service access, support safe and sustainable discharge home, and provide a trusted resource for clients seeking guidance and resolution. Working closely with healthcare, community, and internal partners, you will help improve client experiences, strengthen trust, and support positive outcomes.

Key Responsibilities
WEGHO Program Support
- Coordinate intake, referrals, eligibility assessment, onboarding, service access, and follow-up for WEGHO clients to support safe transitions from hospital to home.
- Collaborate with hospital discharge planners, care coordinators, healthcare partners, community agencies, and internal teams.
- Conduct home visits, risk assessments, wellness check-ins, and service adjustments to support recovery and monitor progress.
- Connect clients with appropriate internal and community-based support.
- Contribute to cutting-edge partnerships and service improvements within funding parameters to support program goals.
Client Advocacy, & Resolution
- Support clients in understanding their rights, available resolution pathways, and organizational processes.
- Support early issue resolution, relationship repair,
and informal mediation to support relationship building.
- Promote trauma-informed, client-centered, and equitable approaches that strengthen trust and positive client experiences.
Documentation & Collaboration
- Document concerns, outcomes, service barriers, and next steps in a clear, objective, and timely manner.
- Identify and escalate concerns as appropriate, providing clear information to support decision making.
- Monitor recurring issues and to support service improvement, training, policy refinement, and organizational learning.
- Travel to client homes, hospital sites, community locations, and locations as required.
- Maintain professional, respectful communication with clients, families, staff, management, and partners.
We are looking for someone who:
- Has a four-year university degree in Social Work or equivalent required; Disability Studies is an asset.
- Has experience in a social service environment as an employee, student, volunteer, or support person.
- Builds trust and works effectively with diverse clients and stakeholders
- Communicates with empathy while navigating complex or sensitive situations
- Can balance shifting priorities throughout the day.
- Proficiency with Microsoft Office; experience with AlayaCare, Cerner,
or similar systems is an asset.
- Knowledge of relevant legislation and client rights, including the Long-Term Care Act, Ontario Disability Supports Program, and Consumer Bill of Rights.
- Ability to meet hospital-mandated requirements, including required vaccinations or documentation.
